Innovation Award entries will be accepted by email. Submissions on disk will
also be accepted but email is preferred. Please follow the guidelines below in preparing
your materials:

Required file formats for Abstracts and Innovation Reports:
· Microsoft Word 2000/XP/97
· Acceptable File Formats for Supporting Materials
(in addition to the above)
· HTML, Adobe Acrobat (PDF), MS Excel,
MSPowerpoint, MS Publisher and JPG or
GIF for
graphics
· Acceptable Disk Types
· 3.5 inch diskettes
and CD ROM
· Format Requirements
· 12 point type

How
to Submit Your Innovation Project
Read through the complete
set of instructions in the section entitled "Preparing your Abstract"
To ensure anonymity in
the review process, proposals will be submitted to the review committee blindly. Please eliminate
all identifiers (ie. facility names, exact geographic locations, names of
coworkers, etc.) from the abstract and report sections of your application.

Innovation Abstract (hard
copy and disk)

Send your comnplete Application Package, including all
